Fountainview Academy requires several registration/release forms to be filled out by parents/guardians and students and returned to us at the latest by the end of registration day. Without these completed and signed forms, you will not be able to register.
For returning students: (Scroll down for returning student forms) If there are any desired changes to the previous school year’s consent or activities forms, please re-fill out the form (found in the Newly Accepted Students area) that is relevant to the desired area of change and re-submit it to Fountainview Academy.
